DUTIES AND RESPONSIBILITIES:
* This is a specialized clerical work which requires independent
decision making in a very high volume area. The work involves such
activities as coordination of patient flow, appointments and
medical record administration. The incumbent must be able to use
independent judgment and initiative as well as interact
professionally with clients, staff and the public. This
professional work will interview applicants to determine initial
and continuing eligibility in the Aspects Program. * Interview
clients and complete eligibility assessment to determine their
current income and eligibility into the Aspects Program, Ryan White
Part A, Part B and ADAP. Enter all client eligibility information
into HMS, Provide and Care Ware. Responsible for second and/or
third opportunity of accurate insurance/payer verification and
update. Responsible for verifying, updating and notating in HMS
system as appropriate the insurance/payer information and
demographics. * Complete referral determination assessment and
direct clients to needed core medical or support services.
Coordinate, track and follow up with referrals to assist eligible
clients to obtain access to other public and private programs for
which they may be eligible; Medicaid, Medicare, State Pharmacy
Assistance Programs and other state or local health care and
support services. * Determine the need of medical transportation
and food services and facilitate access to appropriate methods.
Maintain and keep track of bus passes and food cards log. Conduct
bus passes and food cards inventory monthly. * Responsible of
answering telephone calls from clients and community partners.
Responsible of reminder appointment calls and to clients that
missed their appointments and reschedule. * Responsible of
monitoring clients with eligibility to expire or expired within 30
days, contact clients and schedule appointments. Document three
attempts to contact client and refer to Early Intervention
Services. * Responsible for timely and correct submission of
timesheet and EARS. Responsible of keeping correspondence current
through emails and voicemail. Participates in staff meetings and
complete all trainings required by DOH.
Incumbent may be required to work before, during and/or beyond
normal work hours or days in the event of an emergency. Emergency
duty required of the incumbent includes working in special needs or
Red Cross shelters, or performing other emergency duties including,
but not limited to, responses to or threats involving any disaster
or threat of disaster, man-made or natural.
Participation in the direct deposit program is a condition of
