lives. What will you do? As a Case Manager at Aspire Health
Partners, your job purpose will be to provide a variety of
essential functions to ensure effective operations of assigned
programs. These functions include: - Providing screening, referral,
emergency, crisis and case management services, participating in
clinical treatment team. - Conducting intake assessments to
determine family needs, efficiently documenting all activities,
conducting activities with the client and families, providing
regular input to the treatment plan. - Attending community meetings
to advocate for client needs, providing support and crisis
intervention when necessary. - Documenting clinical records and
additional program specific documents while maintaining the
integrity of the client. - Ability to use program technology to
communicate effectively with staff. - On occasion, act as the
authorized client transporter for necessary treatment and services.
Qualifications - Bachelor's Degree in Social Work, Psychology or
Human Services related field. - Level II Background clearance- All
Aspire Health Partners Internships and Careers require Level 2
