In this role, you will be responsible for assessing patient needs, planning and administering personalized physical therapy programs, and ensuring continuous improvement in the patient's functionality and mobility. This position offers a highly collaborative environment where impactful work is done every day. Conduct patient assessments and evaluations to determine physical condition and needs. Develop treatment plans based on each patient’s needs and physician recommendations. Implement physical therapy interventions tailored to patient needs, monitoring response and modifying accordingly. Document patient care and progress collaboratively in patient and department records. Educate patients and families on treatments and physical therapy strategies. Maintain patient confidence and protect operations by keeping information confidential. Strong skills in physical therapy modalities and therapeutic procedures Excellent interpersonal and communication skills Ability to work independently and as part of a multidisciplinary team Expertise in rehabilitative care for a wide range of physical conditions Medical Insurance Dental Insurance Vision Insurance 401k Valid Physical Therapy license in the state of practice Current CPR certification preferred Prior experience in inpatient or outpatient rehabilitation settings is advantageous
