Description SHIFT: Weekends OnlySCHEDULE: Full-timeLead Case Manager (Registered Nurse)Schedule: Full-time, WEEKENDSPOSITION SUMMARY:The Lead Case Manager (RN) provides day-to-day supervision to the Case Managment staff, ensures delivery of quality case management services and contributes to the continuity and effective operation of the department. Develops, initiates, and coordinates individual patient care management plans to ensure approprioate level of care across the continuum. Ensures plan meets patient's clinical, psychological and discharge needs in collaboration with the attending physician and interdisciplinary team. Acts as an education resource and provides consultation to hospital medical personnel regarding discharge planning process and applicable federal, state and local regulations; identify benefits, implications and limitations of home care as appropriate. Monitors and controls the use of healthcare resources to achieve desired patient outcomes, decrease length of stay, and decrease resource utilization. Identifies and documents delays in care and services and reports findings to department director. QUALIFICATIONS:* Nursing Diploma or Completion of related health care program required* Florida Registered Nurse (RN) license required;--or approved multi-state license* Minimum of three years recent acute clinical practice or related health care experience* Minimum of two--years Case Management experience preferredSKILLS, KNOWLEDGE & ABILITIES:Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills, Demonstrates analytical and critical thinking abilities with pro-active decision-making and negotiation skills, strong knowledge of governmental and private organization and community resources.Osceola Regional Medical Center is a Joint Commission accredited 404-bed hospital that builds on a proud tradition of healthcare excellence in Osceola County, Florida.
